I am using roon arc ios app to download a smart playlist. When new songs are added to that playlist, roon arc shows them as downloaded even if they are not. I can verify that when playing them they stream for the server. They also appear as unavailable in offline mode (either app offline mode or ios airplane mode).
This does not happen consistently, but I think it happens more consistently if the changes to the smart playlist do not happen in ARC. Sometimes, ARC actually downloads the new songs, sometimes it doesn't. Making changes to the smart playlist in roon arc app seems to trigger the download of missing tracks.

Unifi router forwarding roon arc port. Roon installed in a custom linux box with direct wired access to the router.
We’ve attempted to pinpoint a failing download event in diagnostics - we do see timeouts logged within the ARC framework when it attempts to ping the server for unrelated background functions. However, we’re not seeing Smart Downloads failing or occurring at all within the logged period, likely because we’ve not simply failed to capture the events you’re describing within the timeframe.
Can you please approximate the time/date when you last made changes to Smart Playlists that should have pushed through to ARC and didn’t? We want to validate whether the phone is downloading the tracks at all, or if these requests fail on the server side.

So this reoccurred again on 19th of October at 17:51 (London Time). One of the smart playlists had new tracks which appeared downloaded in in arc, but then showed up as unavailable when switching the device to airplane mode.
When I triggered a change to the smart playlist they all started downloading again.
