No. It’s not an hardware issue. It is present on two different internal sata Hdds and they work perfectly,
Interestingly, today I imported ALAC files and they were all immediately recognised and added to Roon as previously used to happen. No missing tracks, no server reboot needed. Same files ripped to FLAC and WAV to the same drive threw up the same issues and were only added after a server reboot. Definitely does not appear to be a hardware issue.

There should be something with the file format. Don’t know if it’s a global bug or only some configuration has it.

I get this from time to time. Especially when adding a lot of files at once.
Recent example (last week). I have been adding albums of DSF files to the library. When they are 2ch (smaller files) Roon seems to add all the tracks. When the 6ch version (larger files) are added, it seems to pick up only the first few tracks.
to fix, I change the name of the enclosing folder on my hard drive. This seems to prompt Roon to rescan the tracks and they all get added. Annoying, but it works consistantly when I see an album hasn’t been added completely. (I literally just add a character to the folder name. Wait a few seconds, check Roon to see all the tracks are there. Then remove the extra character from the folder.)
Just noting that I have the exact same issue so watching this thread for updates. I have files on an NAS that’s watched by a core on a Windows PC. I am bumping up against the memory limits (Task Managers says roon is using 90% of available memory pretty consistently - with spikes to higher) so I wonder if it is connected to the core not being able to read / write to the database in a timely enough fashion?
Same observations for the last couple of months. As a rule, it happens when adding files of large size (24-96 and above or DSD).
Lately I try to add files one by one, it more or less saves the situation, although dropouts still occur.
It seems that Roon starts a full library scan when adding files (judging by my NUC’s fan whistles when doing so) and gets stuck.
My Roon server does it for every files (usually they are flacs), regardless of their size; 16-44, 24-96, and so on.
Sometimes it adds a few files, sometimes nothing.
A reboot is the only solution I found.

Edit: I added three albums and it worked. After that it resumed the usual behaviour. Only one or a few tracks were added.
So sad.
The reason for the skipped files is always the same: I/O failure.
