Not necessary, but you can add more than one image like cover, back cover, and so on. I often do that, or even add whatever other pictures I want like concert ticket scans or whatever.
There is a naming convention that helps with the order they are displayed:

Add the images (and if you have, pdf files) >>> into the album folder. That’s btw something I recommend to do with the main cover, too – that way the work you put into cover art is easier to back up and also usable outside of Roon.
Something like:
front.jpg (or .tif(f) / .png) … for the front cover (this would also be picked up by Roon as the cover image)
back.jpg … for the back cover
…
sleeve.jpg … for the record sleeve
…
booklet.pdf … for booklets
inside …
When you have lots of images stored next to your media, we take note of the file names.
So, if you have back.jpg, we’ll show that right after the front cover, and you can find the album using the “Has Back Cover” focus.
I don’t remember the whole spec, but we do something similar with images like booklet2.jpg, booklet3.jpg, etc.
Point being, there is a difference between “Has Liner Notes” and “Has PDF”.
