Most often, the underlying cause is a retained exif tag in the artwork file itself. We’d be happy to take a look at the file in our next QA sync after the weekend (either the track with artwork, or the artwork itself). This piece of information, however, does make me suspicious that @AndyR’s caching hypothesis is correct. We’ll keep an eye out for your response and are standing by to investigate logs more deeply.
