That’s a C-Extension, used to be able to play orchestral parts (Mahler, Wagner) which go below the low E.

A lot of European orchestras play 5-string basses – B E A D G. As far as I know they were the primary way in which bassists played the low notes in the late Romantic repertoire. Most orchestral 5 strings are huge so if a player wants a more moveable and versatile instrument that can play an occasional low note, a C extension is the ticket.
